Serious bug with rendezvous autopilot

Circumstances were that I was building a space station at an 8000 km orbit above Kerbin. I was rendezvousing a craft with the station, so the autopilot moved to an ~ 9000km orbit, which happened to cross Mun SOI before the Hohmann transfer node. The autopilot gui reported that I needed to be in the same SOI as the target, but did not release control over the craft. The 'disable autopilot' button was also not present in the window with the message, the autopilot kept the time warp at 10000x, and any attempt to cancel the node it was warping to (which would be post-impact with the Mun) caused it to reschedule the same node at the exact same time over again. I was able to survive by using the node planner to quickly clear all nodes and schedule a circulation before the autopilot rescheduled the "transfer", so I was safe at least for the time being, until I could work out how to get out of Mun SOI and turn the autopilot off.

Clearly, there are a number of things that went wrong here:
- the autopilot should have avoided the SOI transfer.
- the autopilot should have turned itself off when it encountered the unexpected SOI transfer.
- the autopilot should have turned itself off when its next maneuver was cancelled.
- The autopilot should have left the "disengage autopilot" button available in the window.

However, in this case, all the slices of swiss cheese lined up, and I very nearly crashed.

Please fix these problems.
